The small-spotted aloe occur as single rosettes or form sometimes small groups, have short stems and the leaves are long, narrow, with a reddish-green colour. The leaves turn more reddish when the plant is under stress in dry seasons. The leaves are covered with numerous white spots and sharp, reddish-brown triangular teeth are found on the margins of the leaves.
The inflorescences are simple with a single, oblong, narrow raceme. Up to three are produced simultaneously with bicoloured racemes and the dull red buds turn yellow when the flowers open. The flowers produced from May to July are tubular and slightly swollen in the middle. The flowers attract Sunbirds and bees to the garden and the flowers are pollinated by small insects.
